Victoria - July 22, 2004 - The Vancouver Island Health Authority’s (VIHA) Chronic Disease Initiatives project, featuring an application built by Number 41 Media, has been awarded the 3M Health Care Quality Award in the Non-Acute Care Facilities category. The award recognizes innovative health care teams who apply quality management processes to create measurable benefits in the delivery programs and services.
“Although our Chronic Disease Management application is just one part of the initiative, it’s great to know that our work is helping VIHA deliver awarding-winning health care,” said Charles Haigh, director of business development at Number 41 Media.
The Chronic Disease Initiatives project was implemented to help physicians in Victoria, British Columbia better manage the care of patients with diabetes, congestive heart failure, and depression. Number 41 Media worked in collaboration with the VIHA team and the Ministry of Health to create a web-based information system that assists physicians with the consistent application of chronic disease management best practices. The application enables the development of patient registries so doctors can track and recall patients for preventative tests and treatments; strict adherence to treatment protocols; development of multidisciplinary teams; and delivery of education and self-management resources for patients.
“We have proven that good chronic disease management is not possible without a good information system. That information system is key,” said Dr. Art Macgregor, the VIHA medical lead for the project.
The 3M Health Care Quality Award is presented annually by the Canadian College of Health Service Executives to one Canadian project each in non-acute and acute health care areas. The award will be formally presented to the Vancouver Island Health Authority in Victoria in September.
